Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Geriatric Care Coordinator. In this role, you will be responsible for monitoring resident Minimum Data Set (MDS) to ensure accuracy and collaborating with the Interdisciplinary Team (IDT) to determine resident potential for improvement and update care plans.

Responsibilities
  • Collaborate with IDT members to develop and implement individualized care plans that meet the unique needs of each resident.
  • Communicate effectively with managed care/insurance providers, residents, and families to ensure seamless care coordination.
  • Conduct regular care plan conferences with IDT members, residents, and families to review progress and make adjustments as needed.
  • Monitor and communicate changes in the minimum data set process to administration and IDT members.
  • Provide education and training to Certified Nursing Assistants (CNAs) and nursing staff on MDS documentation and related requirements.
  • Obtain prior authorizations for new admissions as required and work with Admissions and Medical Records to ensure compliance with certification and re-certification regulations.
  • Work in a safe and alert manner, prioritizing resident care and well-being.
Requirements
  • Graduate of an accredited school of nursing or equivalent education for licensure.
  • Two years of geriatric experience with Medicare A coordination experience required.
Physical Requirements
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements.
  • Frequent use of vision and depth perception for distances near and far, and to identify and distinguish colors.
  • Frequent use of hearing and speech to share information through oral communication.
  • Frequent keyboard use/data entry.
  • Occasional bending, stooping, kneeling, squatting, twisting, and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Rare climbing.
Licenses/Certifications

Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA), Registered Nurse (RN) - Missouri Division of Professional Registration, and other relevant certifications.

Work Environment

This role is a Day Shift position with a scheduled weekly hours of 40.
