Job Title: Clinical Social Worker - Geriatric Care Specialist

Your primary goal will be to promote the general social and emotional well-being of our residents and identify their social and emotional needs related to medical conditions.

Key Responsibilities:
  • Conduct social histories and cognitive assessments to determine the level of care required by residents.
  • Provide follow-up social work visits to newly admitted residents and document these visits within 90 days of arrival.
  • Offer direct social work interventions to residents and their families, promoting their personal adjustment, emotional and social well-being, and accessing facility and community referrals and resources.
  • Document conversations, meetings, and interventions with residents, families, and staff in the clinical chart, adhering to Federal and State regulations and TVA policy.
  • Administer cognitive assessments, such as the Mini-Mental Status Exam, Montreal Cognitive Assessment, or Geriatric Depression Scale, as needed.
  • Achieve certification by the Virginia Department of Social Services to participate in the completion of the Universal Assessment Instrument and development of the Individualized Service Plan within 90 days of hire.
  • Participate in the development and implementation of resident care policies related to psycho-social aspects of resident care issues.
  • Educate residents and families on the importance of completing Advanced Directives.
  • Coordinate and facilitate conferences with families, powers of attorney, and other interested parties to address problems of adjustment, treatment, and caregiving concerns.
Requirements:
  • A BSW or MSW degree from an accredited university; a licensed clinical social worker (LCSW) is preferred.
  • Proficiency in computer skills, with experience in electronic medical records preferred.
  • At least three years of experience in social work, with at least one year in geriatric care.
  • Ability to deal tactfully and effectively with residents, staff, family members, and the public.
