Job Summary:

Responsibilities:
- Provide skilled nursing care to clients in their homes, including wound care, medication management, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each client.
- Collaborate with clients, families, and healthcare teams to ensure seamless care coordination.
- Accurately document client information, including observations, interventions, and evaluations.
- Travel to clients in your local area on a schedule that fits your needs.

Requirements:
- Current nursing license in good standing in New Jersey.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
