Overview

The Advanced Practice Nurse (APN) plays a pivotal role in delivering patient-centered care through a coordinated model that emphasizes leadership, direct clinical practice, consultation, collaboration, coaching, research, and ethical decision-making. This position requires collaboration with a multidisciplinary team to assess, plan, and implement care for individuals with diverse health and safety needs.

Key Responsibilities

1. Conducts comprehensive medical evaluations, treatments, and clearances for applicants and team members in accordance with established company protocols and regulatory standards.
2. Delivers clinical care with professionalism, serving as an advocate for patients and a role model for peers.
3. Ensures clinical practice aligns with the regulations set forth by the NJ Board of Medical Examiners, the Code of Ethics, and current NJ Department of Health requirements, alongside HMH policies.
4. Develops and executes tailored care plans for patients.
5. Facilitates educational programs and support for patients and employers.
6. Reviews and evaluates existing protocols and standards to integrate best practices into clinical care.
7. Applies evidence-based practices to enhance patient outcomes.
8. Administers medications safely to aid recovery from workplace injuries or illnesses.
9. Prescribes medications and recommends over-the-counter options as necessary.
10. Initiates laboratory tests, immunizations, and diagnostic procedures as permitted by regulations and scope of practice.
11. Demonstrates proficient clinical skills in developing effective treatment plans.
12. Collaborates with physicians and engages with other healthcare team members effectively.
13. Exhibits strong communication skills across disciplines to achieve desired results.
14. Maintains accurate, systematic documentation of clinical findings, recommendations, and patient education.
15. Actively participates in continuing education and maintains relevant documentation.
16. Provides culturally sensitive and educationally appropriate information related to patient care.
17. Acts as a clinical resource for nursing staff, patients, and multidisciplinary team members in evaluating complex cases and determining appropriate interventions.
18. Engages in policy and procedure reviews to ensure alignment with current evidence-based practices.
19. Serves as a resource for staff to address clinical and process improvement opportunities that enhance care quality and outcomes.
20. Fosters a collaborative work environment that supports the organizational mission and values.
21. Encourages innovation in clinical practice by seeking opportunities to incorporate new knowledge.
22. Identifies ethical issues in patient care and coordinates interdisciplinary meetings to address them.
23. May float to various clinic locations as required.
24. Performs additional duties and projects as assigned.
25. Adheres to organizational competencies and standards of behavior.

Qualifications

Required Education, Knowledge, Skills, and Abilities:
1. Graduate of an NLN/AACN accredited nursing program.
2. Master's Degree in Nursing.
3. Maintains clinical competencies through ongoing education and practice as mandated by the Board of Nursing and national certification bodies.
4. Relevant experience in a patient care setting.
Preferred Education, Knowledge, Skills, and Abilities:
1. Master's Prepared Advanced Practice Nurse.
2. Minimum of 3 years of professional experience.
3. Strong organizational and computer skills.
4. Effective verbal and written communication abilities.
5. High emotional intelligence, critical thinking, and interpersonal skills.
6. Ability to adapt to changing priorities.
7. Valid driver's license.

Licenses and Certifications Required:
1. NJ State Professional Registered Nurse License.
2. Advanced Practice Nurse Certification (with Master's Degree).
3. AHA Basic Health Care Life Support HCP Certification.
Preferred Licenses and Certifications:
1. CPR certification.
