POSITION SUMMARY
The RN Case Manager acts as a vital advocate for patients, assisting them and their families in navigating their healthcare journeys. This role involves the development of personalized care plans, ensuring efficient resource utilization, and establishing safe discharge strategies.
KEY RESPONSIBILITIES
- Conduct comprehensive assessments of patients to identify clinical, psychosocial, and financial factors affecting their care.
- Collaborate with patients and families to set meaningful health goals.
- Coordinate services among community organizations, healthcare providers, and family members involved in the patient's care.
- Provide patients and families with essential resources and tools to facilitate informed decision-making.
- Develop care plans that align with the patient's preferences and available resources.
- Ensure effective communication during transitions of care, involving all relevant stakeholders.
- Identify community resources and advocate for solutions to address gaps in care.
- Maintain compliance with healthcare regulations and accreditation standards.
- Participate in multidisciplinary team meetings to discuss patient care and progress.
- Educate patients and families about the financial implications of their care options.
- Monitor resource utilization and track delays in care to enhance efficiency.
- Engage in performance improvement initiatives to enhance care delivery.
QUALIFICATIONS
- Associate Degree in Nursing required; Bachelor of Science in Nursing preferred.
- Minimum of one year of experience in case management, discharge planning, or as a Registered Nurse in a clinical setting.
- Current Registered Nurse license in the state of Florida.
- Basic Life Support (BLS) certification required.
- Case Management Certification is preferred.
