We are seeking a skilled Part Time LPN to join our team at Hubbard County Heritage Living Center.
Job Responsibilities:
- Provide nursing care to residents/patients within the scope of practice of a Licensed Practical Nurse (LPN).
- Coordinate and direct resident/patient care, ensuring seamless transitions and effective communication with other departments/providers.
- Administer medications and complete treatments as ordered, observing and monitoring resident's condition and reporting changes as appropriate.
- Maintain a clean, sanitary, safe, and efficiently organized work area, adhering to infection control policies and procedures.
- Collaborate with Registered Nurses to complete assessments, documentation, and data collection within scope of practice and act timely on findings.
Requirements:
- Degree from an accredited school of nursing for the Licensed Practical Nurse program.
- Current LPN licensure in the state in which they are working.
- Up to 2 years' experience as LPN.
- Ability to communicate effectively in both written and verbal formats.
- Basic computer skills.
- Ability to follow all safety rules, regulations, policies, and procedures of the facility.
Preferred Qualifications:
- Previous experience providing senior care/healthcare setting.
