  • Provide skilled nursing care, including vital sign measurement, IV administration, and wound care
  • Collaborate with the patient care team, including physicians, therapists, and social workers, to ensure comprehensive care
  • Provide patient and family education on disease management and health promotion
  • Document patient progress and communicate with the care team as needed
Requirements
  • Current LPN licensure in the state of New York
  • Experience in home health care or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
