Key Responsibilities- Assess client needs and develop personalized goals to promote independence in the community.
- Collaborate with clients and team members to explore alternative solutions to hospitalization, higher levels of care, homelessness, and incarceration.
- Provide skills teaching and advocacy to support clients' needs, desires, and best interests.
- Utilize critical thinking to make independent judgments and maintain responsibility and accountability for service provided.
- Meet established revenue targets and continue professional development to enhance service provision.
- High school diploma or GED.
- One year of experience with the population served (preferred but not required).
- Valid driver's license when driving any vehicle for work-related reasons.
- Ability to speak, read, and write fluent English.
- Ability to enter documentation in an EHR and use Microsoft Office.
