Job Title: Home Health Aide

The VITAS Healthcare Home Health Aide plays a vital role in providing personal care and support to hospice patients in their homes. As a member of our interdisciplinary team, you will work closely with our nurses and other healthcare professionals to deliver high-quality care and enhance the patient's quality of life.

Responsibilities:
  • Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming.
  • Assist with meal planning and preparation, as well as ambulation and transfer assistance.
  • Perform various treatments as ordered by the physician and supervised by the nurse, including changing colostomy bags and applying ice caps or collars.
  • Monitor and record patient vital signs, such as temperature, pulse, and respiration.
  • Assist with maintaining a safe and healthy environment, promoting patient mental alertness, and providing emotional support to patients and their families.
  • Report any changes in patient condition or home situation to the VITAS Nurse and interdisciplinary team.
  • Teach basic personal care to primary caregivers and practice infection control measures.
  • Document patient status and care provided, and complete at least 12 hours of in-service training per calendar year.
Requirements:
  • High School graduate or equivalent preferred.
  • Meet personnel qualifications of a Home Health Aide as specified in 42 CFR.
  • Documented evidence of 75 hours of training as specified in 42 CFR.
  • Documented evidence of successful completion of a Competency Evaluation program as described in 42 CFR.
  • State Allowable Training/Certification.
