Case Manager - School Based

Are you passionate about working with children and youth, making a positive impact on their lives, and helping them achieve their goals? Lighthouse Youth & Family Services is seeking a highly motivated and experienced Case Manager to join our Outpatient Behavioral Health team.

Key Responsibilities:
  • Provide community-based mental health case management services to youth, adolescents, and young adults.
  • Collaborate with other professionals to provide comprehensive treatment services in areas such as mental health, employment, education, and housing.
  • Develop and maintain accurate and current documentation for client files.
Requirements:
  • Bachelor's Degree in Social Work or a related field.
  • Experience working with youth, adolescents, and young adults with mental health issues and concerns.
  • Excellent written and verbal communication skills.
  • Valid driver's license and good driving record.
