Job Summary:

The Veterans Housing and Recovery Program aims to address the challenges faced by homeless veterans, providing them with a fresh start at independent living. As a Case Manager, you will conduct individual meetings with program veterans, provide daily living skill classes, and offer case management services with a focus on employment and training.

Key Responsibilities:
  • Market services and receive referrals of customers with diverse needs, applying professional social work counseling skills to resolve their issues.
  • Conduct assessments of customer needs through interviews, collateral contacts, and investigations, developing a casework plan and performing agreed-upon services.
  • Provide case management services, coordinate social, community, and mental health services, and establish effective public and working relationships.
  • Maintain detailed case records, prepare statistical reports, and make recommendations on case disposition.
  • Attend staff development programs, including in-service training, staff meetings, and professional seminars.
Qualifications:
  • Bachelor's degree in social work, sociology, psychology, or a related field, or an equivalent combination of education and experience.
  • Two years of professional work experience, with comprehensive knowledge of counseling and casework principles and methods.
  • Considerable knowledge of welfare, health, mental health, and community resources, as well as federal, state, and local rules and regulations governing social services.
  • Ability to provide professional social work treatment, organize and plan work effectively, establish and maintain relationships, and prepare detailed reports.

Certifications and Licenses:

Must have a valid driver's license, reliable transportation, and a satisfactory driving record. State certification as a Social Worker (CSW) or Certified Professional Counselor (CPC) is preferred.

Travel:

Ability to travel on day trips up to 80%, with some overnight travel possible.
