The Role

Position Summary:
This role involves the comprehensive coordination of an assigned group of clients and caregivers. The primary focus is to ensure that client schedules are complete, caregiver schedules are optimized, and that all parties receive the highest standard of service. Regular communication with clients, their families, case managers, and AHC staff is essential to maintain continuity and coordination of care services.
Key Responsibilities:
- Manage and coordinate schedules for a designated group of clients and caregivers.
- Address and resolve concerns related to client and caregiver scheduling.
- Serve as the primary contact for new referrals/intakes, including service introductions, scheduling, and follow-up to ensure satisfaction.
- Monitor service change logs to guarantee compliance and accuracy.
- Identify client authorizations and align them with caregivers possessing the appropriate skill sets.
- Confirm all schedules, including any modifications, with clients and relevant parties.
- Ensure nursing introductions and orientations are scheduled effectively.
- Report any changes in client status, such as hospitalizations or transfers, to the appropriate individuals.
- Provide guidance to caregivers regarding punctuality, dress code, and client-related concerns, documenting findings and escalating issues as necessary.
- Collaborate with the nursing team and supervisors regarding client and caregiver needs.
- Participate in recruitment and onboarding activities, communicating staffing requirements.
- Provide on-call coverage during nights and weekends as needed.
- Perform additional duties as assigned.
Qualifications:
- High school diploma or equivalent.
- Preferred: One or more years of scheduling experience.
- Strong background in customer service.
- Excellent verbal and written communication skills, with the ability to engage effectively with diverse individuals.
- Dependable, organized, flexible, and self-directed, with a collaborative spirit.
- Proficient in Microsoft Office and Teams.
- Sound judgment and decision-making skills, with the ability to multitask in a fast-paced environment.
Physical Requirements:
- Ability to remain in a stationary position for extended periods.
- Occasional requirements include standing, walking, and using hands for various tasks.
Work Environment:
- Office setting.
- Potential travel to other office locations.
