About the Role

The National Health Foundation's Recuperative Care Program is dedicated to providing high-quality post-hospital care, empowering individuals to achieve optimal health and well-being. As an LVN Medical Coordinator, you will play a vital role in shaping the lives of our guests, fostering a culture of compassion, and driving positive outcomes.

Main Accountabilities
  • Develop and implement individualized care plans, ensuring seamless transitions from hospital to home or other care settings.
  • Perform regular medical assessments, monitor vital signs, administer medications, and provide essential education to guests.
  • Maintain accurate and up-to-date records within our CRM database, ensuring timely data analysis and informed decision-making.
  • Cultivate strong relationships with internal stakeholders, external partners, and referring agencies to ensure effective coordination and collaboration.
  • Stay current on industry developments, best practices, and relevant regulations, applying this knowledge to enhance the quality of care provided.
Key Qualifications
  • Active LVN licensure in California.
  • AHA BLS certification (Basic Life Support - CPR/First Aid/AED).
  • Recent work experience with homeless populations, preferably in a healthcare setting.
  • Strong understanding of substance abuse, mental health, and social determinants impacting vulnerable populations.
  • Effective communication, interpersonal, and problem-solving skills.

This position comes with a salary of $ $58,000, depending on experience, along with a generous benefits package, reflecting our commitment to supporting the overall well-being of our employees.
