The Licensed Practical Nurse (LPN) plays a vital role in providing high-quality care to residents in our long-term care facility. As a member of our interdisciplinary care team, you will work closely with healthcare professionals to deliver comprehensive care that meets the unique needs of each resident.
Key Responsibilities:- Provide direct care to residents, including administering medications, performing treatments, and documenting resident interactions.
- Contribute to the assessment of residents and develop individualized care plans that address their physical, emotional, and social needs.
- Collaborate with the care team to ensure seamless transitions of care and effective communication.
- Provide education and support to residents and their families on prevention and treatment plans.
- Participate in quality improvement initiatives to enhance the quality of care and services provided.
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with the State Nursing board and/or possess multi-state licensure privileges.
- Obtains and subsequently maintains required department-specific competencies and certifications.
