Job Summary:
Acts Retirement-Life Communities is seeking a skilled Hospice Social Worker to join our team. As a Hospice Social Worker, you will be responsible for providing individualized social work assessment, counseling, and casework services to hospice participants and families.
Key Responsibilities:
- Develop and implement individualized social work plans to meet the unique needs of hospice participants and families.
- Provide counseling and support to hospice participants and families on issues related to end-of-life care, grief, and bereavement.
- Collaborate with the Interdisciplinary Team (IDT) to ensure comprehensive care and support for hospice participants and families.
- Document and evaluate the effectiveness of social work services provided to hospice participants and families.
Requirements:
- Master's degree in Social Work (MSW) or related field.
- Current licensure in the state where services are rendered.
- Minimum of two years' experience in hospice or a transferable, clinical venue of social work practice.
