Job Title: Occupational Therapist

A dynamic and compassionate Occupational Therapist is sought to join our team at a Day treatment facility located in Flushing, NY. This is a contract position with a schedule of Monday to Friday, 09:00am-05:00pm.

Job Purpose:

The Occupational Therapist will plan and administer Occupational therapy treatment programs for medically referred participants to restore function, relieve pain, and prevent disability following disease, injury, or loss of body parts.

Key Responsibilities:
  • Evaluate participants referred by providers and administer treatment programs in collaboration with providers.
  • Complete assessments, progress, treatment, or revisit summaries, and interdisciplinary notes according to departmental policy and procedure.
  • Perform participant evaluations and treatment at PACE sites and/or in the field (participants' homes, nursing homes, etc.).
  • Maintain a clean, safe environment and follow safety protocols.
  • Participate in developing participant care plans with other interdisciplinary team members and participate in care plan meetings in accordance with state and federal regulations.
  • Document all pertinent participant information as required, including evaluations, progress notes, MDS, and care plans according to departmental policy and procedure.
  • Document all statistical data as required.
  • Consult with physiatrists and other professionals when appropriate.
  • Maintain high standards of healthcare and clinical practice through attendance at continuing education in-services/seminars and reading professional journals.
  • Participate in clinical education of student affiliates.
  • Provide clinical instruction to COTAs.
  • Conduct clinical instruction of Level I & Level II Occupational Therapy, COTAs, COTA students, as well as student volunteers.
  • Recommend equipment for discharges as appropriate.
  • Participate in delivery of in-service training programs for site and departmental staff.
  • Participate in Performance Improvement monitoring.
Qualifications:
  • Education: Successful completion of an Occupational Therapy program from an accredited university or equivalent. Bachelor's Degree required.
  • Experience: One (1) year of experience as an Occupational Therapist and/or experience with Geriatric population preferred.
  • License Registration: New York State licensure as a Registered Occupational Therapist.
  • Language: Bi-Lingual Preferred depending on region (participant demographics and location of PACE sites).
Physical Requirements:
  • Ability to lift and transfer participants.
  • Ability to bend, kneel, reach, stretch, stand, and lift up to 50 pounds.
Other Requirements:
  • Ability to establish rapport with patients and staff.
  • Ability to use all OT Modalities/equipment.
  • Ability to travel to participants' homes, nursing homes, and sites/clinics.
  • Knowledge of PACE regulations.
  • Knowledge of Care Compass and other EMRs preferred.
