Nurse Care Coordinator

Job Summary:
The Plaza Assisted Living is seeking a skilled Nurse Care Coordinator to join our team. As a key member of our nursing department, you will be responsible for overseeing the daily operations, medication pass, supervision, training, and support in nurse delegation duties.
Key Responsibilities:
- Assist the Director of Nursing/Assistant Director of Nursing in ensuring optimal care and services for residents.
- Oversee the nurse delegation program and day-to-day nursing assessments.
- Perform assessments for each resident, prior to admission, 30-days after admission, every six months thereafter, or as needed for any status change.
- Provide assistance on functional assessments, including Fall Risk Assessment, Mini-Mental Assessment, Geriatric Depression Scale, and/or Elopement Assessment.
- Notify the Director of Nursing and Administrator, physician, and family when significant changes in condition occur and take appropriate action as directed.
- Assist in emergency situations involving residents.
- Responsible for writing Incident Reports in a timely manner and following up with an appropriate investigation.
- Ensure medications are ordered and received from pharmacy in a timely manner and handled in accordance with company policy.
- Ensure appropriate contact with physicians occurs regarding written and verbal orders.
- Function as a floor nurse, as needed, including the passing of any PRN medication.
- Make daily rounds to determine unit effectiveness and identify needs and concerns.
- Identify and communicate trends on the care floor, assist the Director of Nursing and Administrator with a plan of action to follow through.
- Make recommendations regarding policies and procedures to maintain the care program in compliance with state and federal laws and regulations.
- Responsible for ensuring care department sustains compliance with state and federal laws and community policies and procedures.
- Collaborate efficiently with state inspectors regarding surveys and care complaints.
- Assure residents' service department, supplies, equipment, and work areas are clean, neat, and orderly and submit requests for supplies within departmental budgets.
- Maintain good rapport with residents, families, visitors, and community members and resolve resident, family, and community concerns promptly.
Supervisory Responsibilities:
- Oversee daily nurse delegation program for all floors responsible for.
- Schedule nursing and resident care staff to ensure appropriate resident/staff ratio.
- Enforce attendance policies if needed and replace team members as needed.
- Supervise care staff in the absence of the Director of Nursing and provide feedback concerning their performance on an ongoing basis.
- Assure documentation of resident records by care staff is complete and accurate by conducting frequent audits.
- Communicate regularly with care staff to provide information and address concerns.
- Assist with providing new hire and ongoing training to resident care staff/care department and instruct personnel on use of equipment.
Requirements:
- Current State of Hawaii Nursing License (LVN, RN), in good standing.
- Current Tuberculosis Clearance, according to the State of Hawaii Department of Health.
- CPR and First Aid Certified.
