We are seeking a skilled and compassionate Registered Nurse to join our team as a Concurrent Care Nurse. In this role, you will provide high-quality patient care, develop and implement individualized care plans, and collaborate with healthcare providers to ensure seamless care coordination.
Key Responsibilities- Develop and implement comprehensive care plans that address the physical, emotional, and spiritual needs of patients and their families.
- Conduct thorough assessments of patients' medical records and evaluate treatment needs to ensure consistency with overall symptoms and diagnosis.
- Collaborate with clinical staff and case managers to coordinate patient care and ensure effective communication among healthcare providers.
- Provide direct patient care, including routine admissions and as-needed visits, to ensure timely and effective treatment.
- Work with healthcare providers and medical directors to discuss patient care, approve medical determinations, and provide recommendations based on concurrent review findings.
- Graduate of a registered nursing curriculum; BSN or higher preferred.
- Two (2) years experience in a healthcare environment or equivalent position.
- Possession and maintenance of a valid Hawai'i State nursing license.
- Valid CPR certification.
- Oncology and pediatric experience preferred.
