We are seeking a skilled Personal Care and Support Specialist to join our Home Health Hospice team. This critical role requires a caring and compassionate individual who can provide personalized care and support to patients in their homes, promoting comfort, dignity, and quality of life.

In this position, you will work collaboratively with our interdisciplinary team to ensure the physical, emotional, and social needs of our patients are met. You will be responsible for providing personal care, homemaker services, and companionship to patients, using safe and effective techniques to perform daily living tasks, light housekeeping, and other assigned duties.

We offer a competitive salary range of $63,000 - $75,000 annually, based on experience and location (Des Moines, IA). Our comprehensive benefits package includes health insurance, retirement savings, and paid time off, as well as opportunities for growth and development within a dynamic and supportive organization.

To succeed in this role, you must possess a high school diploma or equivalent, with completion of a nursing assistant training program (minimum 75 hours) and valid certification (Iowa or Illinois) preferred. Proficiency in computer applications and smartphone usage is also necessary. Additionally, you must have a valid driver's license when driving any vehicle for work-related reasons, complete Age-specific care and reporting of Dependent Adult and Child Abuse training within one month of employment, and obtain CPR Certification and Mandatory Reporter course completion by state requirements.
