Key Responsibilities
  • Assess and Coordinate Patient Care: Conduct thorough assessments of patients to identify their needs and develop individualized care plans in collaboration with the healthcare team.
  • Facilitate Communication and Coordination: Ensure effective communication and coordination between healthcare team members, patients, and families to minimize fragmentation of services and manage resources.
  • Maintain Current Information on Community Resources: Stay up-to-date on community resources and refer patients to those that are appropriate for their care needs.
  • Work Collaboratively with Healthcare Teams: Collaborate with multidisciplinary teams to secure timely and appropriate transitions to the next level of care.
  • Develop and Implement Discharge Plans: Develop discharge plans in collaboration with patients and support persons, identifying goals that will provide maximum benefit for each patient.
  • Document and Communicate Information: Document and communicate information to the healthcare team to coordinate and maximize care.
  • Participate in Quality Improvement Initiatives: Participate in quality improvement initiatives and identify and refer quality issues for review to the Quality Management Program.
  • Perform Reassessments and Evaluate Progress: Perform reassessments and evaluate progress against care goals and the plan of care, revising the plan as needed.
Requirements
  • Education: BSN or BSN in progress and/or willing to acquire within 3 years of hire or transfer into the position; or Masters Degree in Social Work.
  • Effective Decision-Making Skills: Demonstrate effective decision-making skills, creativity in problem-solving, and influential leadership skills.
  • Excellent Communication Skills: Possess excellent verbal, written, and presentation skills.
  • Computer Skills: Moderate to expert computer skills, including proficiency in Microsoft Office and/or Google Suite platforms.
  • Licenses and Certifications: NJ Licensed Registered Nurse or NJ Licensed Social Worker or NJ Licensed Clinical Social Worker.
