Job Summary:

Community Healthlink is seeking a part-time Acute Care LPN to join our team in Northern Worcester County. As a LPN, you will be responsible for providing high-quality patient care and working collaboratively with our multidisciplinary team to achieve optimal patient outcomes.

Responsibilities:

  • Assess the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, taking and monitoring vitals of clients on the unit, medication administration, and documentation.
  • Renders and supervises care of clients on the unit.
  • Provide appropriate health care and coordinates referrals as prescribed by providers.
  • Records all client information within electronic medical record system daily.
  • Offer nursing consult on emergency service evaluations.
  • Work with clients to coordinate care with their PCP, psychiatrist, and other professionals including but not limited to medication refills and f/u medical appointments.

Requirements:

  • Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.
  • Previous clinical experience working with substance abuse and mental health.
  • All bilingual candidates are strongly encouraged to apply.
