About the Role

As a Licensed Practical Nurse with Fallon Health's Summit ElderCare, you will be an integral part of our interdisciplinary team providing high-quality care to our elderly participants. This role requires a strong commitment to delivering compassionate and person-centered care in a dynamic and supportive environment.

Responsibilities
  1. Implement care plans developed by the Provider and Primary RN in collaboration with the Interdisciplinary Team, ensuring timely and effective delivery of services.
  2. Perform nursing tasks as assigned by your direct supervisor, adhering to your license and ensuring accurate documentation in a timely manner.
  3. Work closely with the nursing staff to coordinate all care for Summit ElderCare participants, requiring occasional or frequent travel throughout the service area.
  4. Collaborate with other healthcare professionals to ensure seamless care delivery, addressing unmet participant needs and promoting a culture of excellence.
Requirements

To be successful in this role, you will need:

  • A valid license to practice as a Licensed Practical Nurse in the Commonwealth of Massachusetts.
  • At least three years of nursing experience in a primary care practice or long-term care facility.
  • At least one year of experience working in a nursing capacity with frail elders.
  • Certification in CPR/Healthcare Provider issued by The American Heart Association or other accredited agency.
