Key Responsibilities:
As a Case Manager, you will be responsible for:
- Participating in the referral matching process to ensure the best possible home match for children and families.
- Developing and implementing individualized treatment plans to support the unique needs of each child.
- Facilitating assessments, referrals, and service delivery to ensure children receive the necessary support and resources.
- Building and maintaining relationships with foster parents, children, and community stakeholders to ensure a collaborative and supportive environment.
- Ensuring compliance with state and licensure regulations, as well as company policies and procedures.
Requirements:
To be successful in this role, you will need:
- A Bachelor's degree in a human services-related field or a related field with relevant experience.
- At least 2 years of experience working with children with emotional or behavioral problems.
- Valid driver's license and limited liability auto insurance coverage.
- Excellent organizational, written, and oral communication skills.
- Ability to navigate and execute multiple software systems, including Microsoft Office and Electronic Health Records.
- Successful completion of all required background checks and drug screenings.
