Housing Stability Specialist

About the Role:
Care Resource is seeking a highly motivated and compassionate Housing Stability Specialist to join our team. As a key member of our team, you will work closely with individuals and families experiencing homelessness, providing essential support and guidance to help them achieve stable housing.
Job Summary:
The Housing Stability Specialist will be responsible for coordinating the assessment and engagement of individuals and families with homeless needs. This will involve coordinating entry into shelters, rapid re-housing, permanent support housing, and referrals to access medical, dental, mental health, substance use disorder care, pharmaceutical, treatment education, and other services needed by Care Resource clients. The ideal candidate will be able to provide housing assistance through available resources, including the homeless continuum of care's coordinated entry system.
Key Responsibilities:
- Liaison and Coordinator: Serve as a liaison, coordinator, and/or advocate between the Homelessness, Housing case Management, Medical Care departments or other community medical or agency service provider to remove Homelessness barriers to House/care for clients.
- Client Support: Accompany clients to Housing, medical, mental health, substance use disorder treatment or social service appointments to improve Housing Stability and medical care.
- Rapport Building: Create rapport within client interaction to help each progress in their Housing Issue to avoid homelessness situation.
- Client Interviews: Interviews prospective clients to determine Homeless need, individual needs, and eligibility for various medical and social services, enrolling them into available community programs.
- Client Tracking: Maintains organized system of tracking client outcomes through the services point systems (HMIS) and follow up with the goal of seeing clients progress toward improvements in their lives.
Services Planning and Documentation:
- Research and Outreach: Conduct research, outreach, education, and public relations to build a pool of landlords and property management companies willing to rent to program participants.
- Medical Coordination: Coordinates, support and follows up on medical treatments.
- Caseload Management: Maintains an average annual active caseload as assigned by the supervisor.
- Application Processing: Assist in the processing and submission of applications for housing (subsidized and unsubsidized).
- Data Entry: Inputs client information using electronic data entry according to agency and departmental guidance.
Physical Requirements:
This work requires the following physical activities: constant sitting, walking, and hearing, talking in person and talking on the phone. Occasional driving, stretching/reaching, and standing are required. Work usually performed in an office setting.
Travel Requirements:
Own transportation required for Home, Airport, streets visits/Annual.
Required Skills and Qualifications:
Two (2) years of related work experience is required. Knowledge of homeless community resources and community services is required. Experience to work with multicultural and diverse population. Experience in Rapid Re-Housing program, Permanent Supportive Housing and FQHC program.
Estimated Salary: $60,000 - $70,000 per year, based on experience and qualifications.
