Job Title: Nurse Practitioner or Physician Assistant

Job Summary:

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team at Matrix Medical Network. As a key member of our clinical team, you will be responsible for conducting comprehensive health assessments, providing medical care, and collaborating with healthcare providers to ensure the best possible outcomes for our patients.

About the Role:

This is a PRN position, offering flexible scheduling and the opportunity to work in a variety of settings, including home visits, skilled facilities, and telehealth. You will have the ability to travel and work in different locations, with a valid state driver's license and adequate automobile insurance coverage.

Responsibilities:

  • Conduct comprehensive health assessments, including medical history, medication adherence, social environment, and other risks
  • Provide medical care, including diagnosis and treatment, health education, and physician referrals
  • Collaborate with healthcare providers, case managers, social workers, and other healthcare professionals to ensure the best possible outcomes for patients
  • Work in a variety of settings, including home visits, skilled facilities, and telehealth
  • Travel to different locations, with a valid state driver's license and adequate automobile insurance coverage

Requirements:

  • Master's degree in nursing or a related field, or commensurate experience and satisfactory completion of NP licensure
  • Current RN and NP/PA licensure in the state of practice, with prescription authority or the ability to obtain prescriptive authority
  • Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
  • Strong computer skills and familiarity with EMR software
  • Excellent verbal and written communication skills
  • Positive and outgoing personality, with a strong team player attitude
