The successful candidate will be responsible for conducting home visits, educating parents on prenatal and postpartum care, and providing information on toddler health and care.

Key Responsibilities
  • Conduct regular home visits to provide education and support to clients
  • Develop and implement individualized care plans to meet the needs of clients
  • Collaborate with community agencies to ensure clients receive necessary services
  • Document client interactions and progress in a timely and accurate manner
  • Participate in training and quality assurance activities as needed
Requirements
  • Bachelor's degree in Social Science or related field
  • 2-3 years of community-based work experience in community health, community outreach, or social services
  • Community Health Worker Certification (CHW) preferred
  • Proficient in Microsoft Word, Excel, and PowerPoint
  • Valid State of Ohio driver's license and insurance on any personal vehicle used for work
