We are seeking a highly motivated and experienced Community Health Advocate to join our team at Public Health - Dayton and Montgomery County. The successful candidate will be responsible for conducting home visits, educating parents on prenatal and postpartum care, and providing information on toddler health and care.
Key Responsibilities- Conduct regular home visits to provide education and support to clients
- Develop and implement individualized care plans to meet the needs of clients
- Collaborate with community agencies to ensure clients receive necessary services
- Document client interactions and progress in a timely and accurate manner
- Participate in training and quality assurance activities as needed
- Bachelor's degree in Social Science or related field
- 2-3 years of community-based work experience in community health, community outreach, or social services
- Community Health Worker Certification (CHW) preferred
- Proficient in Microsoft Word, Excel, and PowerPoint
- Valid State of Ohio driver's license and insurance on any personal vehicle used for work
- Medical, Dental, Vision, and Life Insurance
- Vacation, Sick, Personal Leave, and Paid holidays
- Tuition Reimbursement
- Membership with Ohio Public Employees Retirement Systems (OPERS)
- Eligibility to contribute to a Deferred Compensation Program
