Job Summary

We are seeking a highly skilled and compassionate Registered Dietitian to join our team at Carlisle Nursing & Rehab in Carlisle, PA. As a key member of our healthcare team, you will be responsible for providing expert nutrition care to our residents, ensuring their optimal health and well-being.

Key Responsibilities:
  • Develop and implement individualized nutrition care plans for residents, taking into account their medical history, dietary needs, and preferences.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and effective communication.
  • Conduct regular assessments and evaluations to monitor residents' nutritional status and adjust care plans as needed.
  • Provide education and counseling to residents, families, and caregivers on healthy eating habits, nutrition, and meal planning.
  • Stay up-to-date with the latest research, guidelines, and best practices in nutrition care, applying this knowledge to improve resident outcomes.
Requirements:
  • Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential.
  • Experience working in a healthcare setting, preferably in a long-term care or geriatric population.
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on providing exceptional patient care.
