Job Summary

We are seeking a skilled Housing Case Manager to join our team at Westhab Inc. The successful candidate will provide housing-focused case management services to homeless families in an emergency sanctuary family shelter. Key responsibilities include formulating housing plans, facilitating tenant education, and referring residents to specific housing opportunities.

Key Responsibilities:

  • Develop and implement housing plans to ensure timely transition to permanent housing.
  • Provide individual and group tenant education on topics such as interviewing skills, lease agreements, and financial literacy.
  • Refer residents to suitable housing opportunities and assist with application processes.
  • Offer crisis intervention services as needed and participate in case conferences.
  • Maintain accurate records of services provided and required documentation.

Requirements:

  • Bachelor's degree in Social Work or Human Services Field preferred.
  • At least 1 year of case management experience working with homeless, mentally ill, substance abuse, and/or low-income populations.
  • Bilingual required, with strong relationship building and organizational skills.
