About the Role

We are seeking a highly motivated and compassionate Service Coordinator to join our Crisis Residential Program team at Sheppard Pratt. As a Service Coordinator, you will play a critical role in providing short-term intensive support to individuals experiencing a mental health crisis.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of client needs and develop personalized service plans to ensure their stability and well-being.
  • Service Coordination: Coordinate client medical appointments, transportation, housing, and other essential services to support their independence.
  • Medication Management: Develop and assist with client medication management plans to ensure safe and effective treatment.
  • Entitlements and Benefits: Obtain and maintain entitlements for clients, including government benefits and other resources.
  • Documentation and Reporting: Maintain accurate and complete documentation of client progress and services provided.
  • Transition Planning: Develop and coordinate transition plans to ensure a smooth discharge from the program.

Requirements
  • Education and Experience: Possess a high school diploma or GED and 2 years of related experience, or a bachelor's degree and 1 year of related experience.
  • Certifications and Trainings: Be willing to obtain additional certifications and trainings as required.
  • Driver's License: Hold a valid driver's license with 3 points or less and have access to an insured vehicle.
  • Availability: Be available to work days, nights, weekends, and holidays as needed by the program.
