### Job Summary

We are seeking a skilled and compassionate Health Navigator 4 to join our team at Broward House, Inc. In this role, you will play a critical part in supporting individuals living with HIV to achieve their health goals.

### Key Responsibilities

* Build strong relationships with clients, understanding their strengths, needs, and preferences
* Collaborate with clients to develop personalized plans of care, incorporating input from medical teams
* Arrange services and complete linkages based on individualized needs
* Provide coaching and education to enhance clients' understanding of disease progression, treatment, and healthy lifestyles
* Coordinate with internal and external interdisciplinary treatment teams to ensure seamless client services
* Conduct home visits to provide personalized support
* Promote program refinement and effectively participate in assigned meetings

### Requirements

* Licensed Practical Nurse with preferred experience in case management/social services
* Ability to effectively communicate with people from diverse backgrounds
* Solution-focused approach to conflict and challenges
* Moderate to advanced computer skills
* Ability to work independently with minimal direction
* Knowledge of substance abuse and mental health symptoms, as well as basic interventions
* Familiarity with local community resources for patients with chronic diseases
* Ability to communicate clearly and effectively
* Knowledge of patient teaching, health promotion, and disease prevention methods
* Familiarity with Patient-Centered Medical Homes concept
* Ability to legally operate a motor vehicle and provide own transportation
