Case Manager
At Sevita, we believe that every role is essential, and our team is committed to making a meaningful difference in the lives of those we serve.

Facilitate the creation, assessment, and execution of service plans, case management strategies, behavioral intervention plans, and treatments for individuals in our care. Supervise all services rendered to individuals and their families, ensuring high-quality care, adherence to contractual obligations, and compliance with regulatory standards. Contribute to the development of effective services and engaging activities tailored to the needs of those we support. Foster and maintain collaborative relationships with families and external case management professionals.

Qualifications:

A Bachelor's degree in human services or a related discipline. At least one year of relevant experience working with individuals with intellectual and developmental disabilities, brain injuries, or at-risk youth is preferred. A valid driver's license, vehicle registration, and auto insurance are required. Licensure and training as mandated by state regulations, including CPR, First Aid, and Behavioral Intervention Techniques. QMRP/QIDDP certification as required by state guidelines. A dependable, responsible demeanor coupled with a compassionate approach to care. A strong commitment to delivering quality in all aspects of your work.
