As a home health RN case manager, you will have the opportunity to make a meaningful difference in the lives of our patients and their families. Our team is dedicated to providing exceptional care in the comfort of our patients' own homes.

Compensation: $72,000 - $80,000 per year

About the Role

This is a full-time position that requires the ability to work independently and collaboratively as part of a multidisciplinary team. The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to think critically and solve problems effectively.

Responsibilities:

  • Conduct thorough assessments of patients' physical, emotional, and social needs
  • Develop and implement individualized care plans that address patients' health concerns and goals
  • Collaborate with other healthcare professionals to ensure seamless coordination of care
  • Provide education and support to patients and their families on self-care techniques and disease management
  • Document all interactions and interventions in patients' medical records

Requirements:

  • Graduate degree in nursing from an accredited institution
  • Licensed as a registered nurse in the state of Texas
  • At least one year of experience as an RN
