About the Opportunity

We are excited to offer a challenging and rewarding role as a Patient Care Manager at SouthernCare New Beacon Hospice. This position offers a unique blend of clinical and leadership responsibilities, allowing you to make a meaningful impact on the lives of our patients and their families.

About the Position

  • This is a safety-sensitive position that requires a Registered Nurse license with a minimum of an Associate's Degree in Nursing and 3 years of experience in direct patient care.
  • A Bachelor's Degree in Nursing is preferred, as well as a minimum of 1 year of management/supervisory experience and 2 or more years of experience in hospice or home care.
  • You will be responsible for managing the site in the absence of the Executive Director, Administrator, or Senior Patient Care Manager, responding to phone referrals/inquiries, and participating in on-call nursing schedules.

Responsibilities

  • Develop and implement effective strategies to improve patient outcomes and enhance the overall quality of care.
  • Collaborate with interdisciplinary teams to achieve seamless coordination of care.
  • Contribute to the development and implementation of policies and procedures that align with our organizational goals.
