Position Summary:

As an Advanced Practice Nurse at Kingston Healthcare, you will deliver comprehensive primary and specialized healthcare services to our residents. You will work closely with our interdisciplinary team to ensure high-quality, patient-focused care. Your role will be crucial in evaluating, diagnosing, and managing the health needs of our residents, promoting wellness, and upholding the highest standards of clinical practice.

Key Responsibilities:

  • Perform comprehensive assessments of residents' health, including physical examinations, medical histories, and diagnostic tests to develop precise diagnoses and tailored care plans.
  • Oversee and treat both acute and chronic medical conditions, prescribing necessary medications and treatments while monitoring residents' responses to therapies.
  • Educate and counsel residents and their families on health maintenance, disease prevention, and self-care strategies, fostering a collaborative healthcare management approach.
  • Collaborate with a multidisciplinary team, including physicians, nurses, therapists, and other healthcare professionals, to coordinate care, address complex medical challenges, and enhance resident outcomes.
  • Ensure accurate and timely documentation of patient interactions, assessments, treatment plans, and progress notes in compliance with regulatory standards and best practices.
  • Engage in quality improvement initiatives, evidence-based practices, and ongoing education to enhance clinical expertise and contribute to the advancement of healthcare delivery at Kingston Healthcare.

Qualifications:

  • Master's or Doctoral degree in Nursing with a Nurse Practitioner specialization from an accredited institution.
  • Current national certification as a Nurse Practitioner, specializing in adult-gerontology, family, or a related field.
  • Active state licensure as a Nurse Practitioner, with the authority to practice independently and prescribe medications. DEA licensed.
  • Prior experience in primary care, geriatrics, or long-term care settings is preferred; however, new graduates with relevant clinical rotations are encouraged to apply.
  • Strong clinical assessment and diagnostic capabilities, with the ability to manage a wide range of acute and chronic medical conditions in older adult populations.
  • Excellent communication, interpersonal, and teamwork skills, with a compassionate and patient-centered approach to care.
