Role Summary

The Manager of Social Services Case Management is responsible for the development and oversight of social services initiatives within the organization. This role involves establishing and maintaining high standards of service quality, cost-effectiveness, and a focus on customer satisfaction. The position requires collaboration with community service providers to enhance program delivery.

Key Responsibilities:
  • Designs, implements, and maintains policies and procedures for social services and discharge planning.
  • Oversees case management activities within the social services department.
  • Manages team performance, ensuring staff competency and providing necessary training.
  • Identifies and develops new programs aimed at improving services for specific patient demographics.
  • Administers the departmental budget and identifies opportunities for cost savings and service enhancements.
  • Researches and connects community resources to support member needs.
  • Implements quality assurance measures for social services programs and staff performance.
  • Addresses challenges that impact service delivery and formulates effective solutions.
  • Collaborates with various departments and community organizations to ensure integrated service provision.
  • Ensures adherence to all relevant federal, state, and local regulations.
Qualifications:

Experience: A minimum of two years in a leadership or supervisory role within medical social work.

Education: A Master’s degree in social work from an accredited institution.

Licensure:

  • Licensed Clinical Social Worker (California)
  • National Provider Identifier required upon hire
Additional Skills:
  • Strong understanding of fiscal management principles.
  • Knowledge of healthcare systems and human development theories.
  • Able to thrive in a Labor/Management Partnership environment.
Preferred Experience:

Five years of supervisory experience in medical social work is preferred.
