Job Title: Manager Social Services Case Management

At Cedent Consulting, we are seeking a highly skilled and experienced Manager Social Services Case Management to join our team.

Job Summary:

The Manager Social Services Case Management will be responsible for developing and monitoring social services programs for our clients. This includes establishing and maintaining standards, quality levels, cost-effective services, and customer-focused interfaces with community services.

Key Responsibilities:
  • Plan, develop, implement, and maintain social services policies and procedures, delivery of social work programs, and discharge planning services.
  • Oversee social services case management activities.
  • Manage staff activities, ensure their competencies, and develop/present in-services.
  • Recommend, develop, and implement new programs enhancing services to specialized patient populations and reducing hospital and office visit utilization.
  • Develop and manage departmental budget.
  • Identify and implement opportunities for cost reduction and improved services.
  • Identify community resources for our members.
  • Collaborate with community organizations in developing new programs or enhancing current community services which are available to client members.
  • Develop, implement, and monitor the quality assurance of the social services programs and staff.
  • Identify issues which interfere with the delivery of services and develop techniques to resolve them.
  • Collaborate with medical center departments, community agencies, and health plan members on matters related to discharge planning and ensures that services are provided in an integrated fashion.
  • Ensure compliance with federal, state, and local requirements.
Qualifications:
  • Minimum two (2) years of leadership or supervisory experience in medical social work.
  • Masters degree in Social Work from an institution accredited by the Council of Social Work Education.
  • Licensed Clinical Social Worker (California).
  • National Provider Identifier required at hire.
Additional Requirements:
  • Sound knowledge of fiscal management theories and principles.
  • Knowledge of different levels of healthcare; understanding of human development and psychodynamic theories.
  • Must be able to work in a Labor/Management Partnership environment.

This is a full-time position.
