Salary: $85,000 - $110,000 per year, depending on location and experience

About the Job

We are seeking a skilled and compassionate LTSS Care Manager Nurse Specialist to join our team. In this role, you will be responsible for managing member cases within your scope of licensure, providing supervision and direction to non-RN clinicians, and developing, monitoring, evaluating, and revising member care plans to meet their needs.

Responsibilities

  • Identify high-risk members and coordinate care with healthcare teams
  • Manage members with chronic illnesses, co-morbidities, and disabilities, ensuring cost-effective utilization of health benefits
  • Conduct thorough member histories to develop individualized care plans
  • Establish short- and long-term goals in collaboration with members, caregivers, family, natural supports, and physicians
  • Oversee development and implementation of member care plans, ensuring access to necessary services
  • Interface with Medical Directors, Physician Advisors, and Inter-Disciplinary Teams on care management treatment plans

Requirements

  • RN license in applicable state(s)
  • Minimum 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities
  • MA/MS in Health/Nursing preferred
  • Ability to travel to worksite and other locations as necessary
