Job Title: Licensed Practical Nurse

Opportunity Living is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As an LPN, you will provide high-quality nursing care to our clients with developmental disabilities.

Job Summary

The LPN will be responsible for providing nursing services, including assessments, observations, and advice to clients and staff. You will work collaboratively with the Director of Health Services, physicians, and nurses to ensure the best possible care for our clients.

Key Responsibilities
  • Participate in the development, review, and update of individual program plans as part of the Interdisciplinary Team process.
  • Assist the RN with quarterly nursing assessments and gather information and vital signs.
  • Arrange quarterly direct physical examinations of clients, including visual examinations and review of body systems.
  • Refer clients with health problems to physicians and train clients and staff in health and hygiene methods.
  • Schedule routine appointments and accompany clients as needed.
  • Administer medications and treatments, observe for reactions, and maintain accurate patient reports.
  • Evaluate the effectiveness of temporary health care plans and confer with other health care team members.
  • Develop a coordinated plan of care based on assessments and individual needs of clients.
  • Train clients to self-administer medications and provide instant medical care in emergencies.
  • Participate in an established on-call system and maintain flexibility of schedules.
  • Assist in ensuring the effective operation of the organization by performing duties assigned by supervisor/manager.
  • Maintain professional competency through continuing education and personal growth experiences.
  • Protect client and staff confidentiality and follow HIPAA policies.
Qualifications
  • Valid license in the state as a Licensed Practical Nurse (LPN).
  • General knowledge of nursing care methods and procedures.
  • Excellent communication and interpersonal skills.
  • Strong analytical and problem-solving skills.
  • Excellent organizational skills and attention to detail.
  • Ability to follow routine verbal and written instructions.
  • Training and/or experience in developmental disabilities preferred.
  • Commitment to Opportunity Living's Mission and Vision.
  • Ability to communicate with all persons in a culturally competent manner.
  • Ability to pass criminal/registry background checks and maintain required trainings.
