This position requires strong organizational and communication skills, as well as the ability to work independently and as part of a team. The ideal candidate will have a Bachelor's degree in a Human Service related field and at least 2 years of experience working with older adults.

Givens Communities offers a competitive salary range of $65,000 - $85,000 per year, depending on experience. Our benefits package includes health insurance, retirement plan, paid time off, and professional development opportunities.

**What You'll Do:**

  • Coordinate the care and services of residents
  • Develop and implement care plans in collaboration with healthcare professionals
  • Provide referral and community resource information to residents and families

**What You'll Need:**

  • Bachelor's degree in a Human Service related field
  • 2+ years of experience working with older adults
  • Strong organizational and communication skills
  • Database, spreadsheet, and Microsoft Office skills
