Job Title: Registered Nurse OR Educator Quality Analyst

We are seeking a compassionate and skilled Registered Nurse or Educator Quality Analyst to join our team at Kindred at Home. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality care to our patients and their families.

Job Summary:

The Registered Nurse or Educator Quality Analyst will be responsible for providing skilled professional nursing care to patients and their families, as well as identifying and addressing their physical, psychological, social, and spiritual needs. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.

Responsibilities:
  • Provide skilled professional nursing care to patients and their families, as directed by the supervising Case Manager or Patient Care Manager.
  • Identify and address the physical, psychological, social, and spiritual needs of patients and their families.
  • Work closely with other members of the interdisciplinary team to ensure all patient and family care needs are being met.
  • Implement nursing plans of care as directed by the interdisciplinary team and Patient Care Manager.
  • Provide patient and family education and information pertinent to diagnosis and self-plan of care.
  • Participate in coordination of Hospice services, as needed.
Requirements:
  • LPN or LVN licensed in the state where program is located.
  • Graduate of accredited practical nurse or vocational nursing program.
  • Minimum of one year general nursing experience.
  • Community health/Hospice or medical/surgical experience.
  • Understanding of Hospice philosophy and needs of terminally ill.
  • Self-directed with ability to work with little supervision.
  • Demonstrate excellent observation, good nursing judgment, and communication skills.
  • Ability to provide good written documentation in a timely manner.
