Crisis Assessment Social Worker Part Time Days

The Social Worker is a key member of the care team, responsible for facilitating patient care along a continuum through effective resource coordination. This role involves assessing patients for transition needs, identifying and assessing patients at risk for readmission, and conducting complex psycho-social assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.
Responsibilities:- Conduct complex psycho-social transition planning assessments and reassessments, and implement interventions as needed.
- Assist with adoption, abuse, and neglect cases, including assessment, intervention, and referral to local, state, and/or federal agencies.
- Coordinate care and implement or oversee the implementation of transition plans.
- Lead and/or facilitate multi-disciplinary patient care conferences, including Complex Case Review.
- Make appropriate referrals to other departments and communicate with patients and families about the plan of care.
- Collaborate with physicians, office staff, and ancillary departments to ensure patient education is completed to support post-acute needs.
- Timely complete and concise documentation in the Case Management system.
- Maintain accurate patient demographic and insurance information.
- Precept new staff members and act as a resource to all staff.
- Facilitate TEMPO as needed and participate in department quality improvement initiatives.
- Master's of Social Work degree.
- Current licensure, certification, or registration to practice as a social worker in the state of practice or policy.
- Active license for state(s) covered.
- Preferred: 2 years of acute hospital experience and Accredited Case Manager (ACM) certification.
This role requires frequent sitting, moderate standing, walking, reaching, stooping, and bending. Manual dexterity, mobility, and touch are necessary to perform all related duties of the position.
