Job Title: Hospice Social Worker

We are seeking a compassionate and skilled Hospice Social Worker to join our team at Interim HealthCare Home Health and Hospice. As a Hospice Social Worker, you will play a vital role in providing emotional and social support to patients and their families during a challenging time.

Key Responsibilities:
  • Conduct thorough psychosocial assessments of patients and families to identify their unique needs and develop personalized care plans.
  • Provide individual and family counseling to address mental and emotional health concerns, and connect patients with community resources as needed.
  • Collaborate with healthcare teams to ensure comprehensive care and support for patients and families.
  • Identify and address cultural and religious diversity needs, and provide education on available resources.
  • Develop and implement crisis intervention plans to support patients and families in times of need.
Requirements:
  • Master's degree in Social Work from a Council on Social Work Education-accredited program.
  • Active licensure as a Social Worker in the state of employment.
  • Minimum one year of experience in healthcare, with hospice or homecare experience preferred.
  • Reliable transportation and a valid driver's license.
