Job Summary

The Clinical Pharmacist II - Specialty is a key member of the healthcare team, responsible for providing safe, effective, and prompt medication therapy to patients. This role requires a high level of clinical expertise and a strong commitment to patient-centered care.

Key Responsibilities
  • Provide direct patient care, including assessing patient needs, incorporating age and disease-specific characteristics into drug therapy, and adjusting care according to patient response.
  • Participate in all aspects of the medication-use system, including clinical research, quality improvement initiatives, and healthcare provider education.
  • Collaborate with healthcare providers to provide patient-centered care, identify high-risk patients, and implement interventions to improve quality and safety.
  • Oversee the prior authorization and appeals process, and provide clinical assistance to technician staff as needed.
  • Participate in a rotating after-hours on-call process and maintain proficiency in hospital computer systems and medication ordering systems.
  • Provide accurate, safe, timely, and appropriate medication therapy based on patient age and needs, and complete critical patient monitoring and reviews.
Requirements
  • Doctor of Pharmacy degree from an ACPE accredited school of Pharmacy is required.
  • PGY1 residency or 5 years of clinical practice (community, ambulatory care, PBM, or hospital) required.
  • PGY2 residency preferred.
  • Specialty pharmacy experience preferred.
  • EPIC suite products experience preferred (EMR, Compass Rose, Willow Ambulatory).
  • Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine in areas appropriate for clinical assignments required.
  • Licensed pharmacist in the state of practice.
  • Pharmacy preceptor license within 90 days of start date.
  • Must accrue American Society of Health-System Pharmacists (ASHP) Residency Preceptor status within 18 months of hire.
  • Must obtain Certified Specialty Pharmacist (CSP) certification or other applicable board (e.g. BCOP, BCACP, BCPS, BCMTMS, BCGP) certification within 18 months of hire.
