About the Role

We are seeking a skilled Occupational Therapist to join our Home Health team. As a Home Health Occupational Therapist, you will work with patients in their homes to help them recover from illness, injury, or surgery.

Key Responsibilities
  • Assess patients' daily living and work-related skills and develop therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic and prognostic tests to assist physicians in evaluating patients' level of function.
  • Confer with physicians and other healthcare team members to develop and revise treatment plans.
  • Provide therapeutic treatment and instruction to patients to improve strength, coordination, range-of-motion, and function, or teach compensation measures.
  • Review and expand retraining programs to align with patients' behavioral goals and rehabilitation program guidelines.
  • Consult with vocational team members to develop and implement vocationally oriented plans consistent with patients' needs and capabilities.
  • Report patient status and progress to physicians, rehabilitation staff, Clinical Manager, patients, and family members.
  • Participate in care coordination and discharge planning activities and serve as a resource to other healthcare personnel in meeting patients' needs.
  • Design community reintegration activities to assist patients in physical reconditioning and psychological adjustment, and coordinate plans with interdisciplinary team members.
  • Recommend and design special adaptive equipment to improve patients' residence or working environments or participation in the rehabilitation program.
  • Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills, and participate in team and family educational meetings.
  • Coordinate, oversee, supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
  • Accurately and thoroughly document patients' care observations, interventions, and evaluation on the day services are rendered, and ensure that interim orders received from physicians are accurately documented and implemented.
Requirements
  • Degree from an accredited Occupational Therapy Program.
  • A minimum of six months of occupational therapy experience preferred.
  • Home Health experience a plus.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation are required.
