Occupational Therapist Job Description

At Presbyterian Homes and Services, we are seeking a highly skilled Occupational Therapist to join our Outpatient Therapy team at St. Andrew's Village in Mahtomedi, MN and Croixdale in Bayport, MN.

Responsibilities:
  • Provide high-quality occupational therapy care and services to residents, consistent with regulations and established rehabilitation policies and procedures.
  • Develop and implement individualized treatment plans, incorporating resident goals and needs.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support.
  • Communicate effectively with residents, families, and healthcare professionals to ensure seamless care transitions.
  • Participate in quality improvement initiatives and contribute to the development of best practices in occupational therapy.
Requirements:
  • Graduate from an accredited Occupational Therapy program.
  • Current licensure in the state of practice as required.
  • Competence in occupational therapy evaluation and treatment.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.
