Role Summary:

JOB RESPONSIBILITIES:

The Patient Care Coordinator is responsible for managing the care of patients with complex clinical needs throughout their healthcare journey. This includes ensuring high-quality clinical outcomes and effective resource utilization. The coordinator will negotiate and arrange necessary services and resources for patients while addressing barriers to efficient care delivery.

This role requires effective communication with hospital staff, physicians, and payors, ensuring a collaborative approach to patient care. The coordinator will also provide education to patients and families, enhancing their understanding of care processes and available resources.

Key Responsibilities Include:
- Assessing patient needs and determining appropriate levels of care.
- Documenting assessments and interventions in accordance with departmental policies.
- Collaborating with healthcare team members to evaluate clinical interventions and ensure adherence to care standards.
- Initiating referrals to appropriate team members as needed.
- Communicating patient needs during transfers to different care levels.
- Identifying and addressing operational challenges that may hinder care progression.
- Applying utilization criteria to monitor admissions and continued stays, ensuring compliance with guidelines.
- Coordinating discharge planning and ensuring patients have the necessary resources for a smooth transition.
- Engaging with multidisciplinary teams throughout the discharge process, including care planning discussions.
- Facilitating referrals for additional services such as home health care and medical equipment.

Qualifications:
- A minimum of three years of diverse hospital experience or related roles.
- Preferred experience includes three to five years in acute care settings and discharge planning.
- Must be a graduate of an accredited nursing program with an active nursing license.
- Certification in case management (ACM or CCM) is required within one year of eligibility.
- A Bachelor’s degree in Nursing or a related health field is preferred.

All nursing professionals in this role must maintain a valid nursing license as per state regulations.

BLS certification is mandatory.

COMPENSATION:
Competitive hourly rate based on experience.

WORK ENVIRONMENT:
The role may involve exposure to various environmental factors, including but not limited to:
- Blood-borne pathogens
- Bio-hazardous materials
- Chemicals and communicable diseases
- Potential for electrical hazards and varying temperature conditions.

Personal protective equipment is provided and must be utilized as required.
