Job Summary

Catholic Community Services of Southern Arizona, Inc. is seeking a skilled Substance Abuse Case Manager to provide compassionate services to low-income individuals and families with children. As a key member of our team, you will conduct intake and assessment of clients, provide advocacy and follow-up assistance, and maintain necessary case records and client file documentation.

Key Responsibilities
  • Process self-referrals and referrals from community agencies by providing initial screenings, intakes, interviews, and eligibility assessment of participants
  • Develop individual service plans in partnership with clients; monitor progress and implement follow-up activities
  • Provide referrals, assist clients in arranging appointments with community resources or private group counseling sessions, and monitor client progress
  • Maintain individual service files for clients that include plans and strategies for achievement of goals and provide follow-up contacts to assess progress
Requirements
  • Associate Degree plus two (2) years of Behavioral Health work experience; or High School Diploma or GED plus four (4) years of Behavioral Health work experience
  • Excellent data entry skills (speed and accuracy)
  • Proven skills utilizing Microsoft Office Suite
  • Excellent organizational skills
  • Proven strong communication skills - oral and written communications are clear and concise using appropriate style, grammar, and tone
Regulatory Requirements
  • Must be at least 18 years of age
  • Valid driver license, proof of insurance, and 39-month motor vehicle report
  • Ability to obtain and maintain Arizona Level One Fingerprint Clearance Card and FBI National Criminal Records History Report (employer paid)
  • Pass pre-employment drug screen (includes marijuana, regardless of recreational use laws) (employer paid)
Desired Qualifications
  • Bachelor Degree in social service or related field
  • Bilingual in English and Spanish, verbal and written
  • Additional years of directly-related experience
