Job SummaryWe are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Case Manager RN. In this role, you will be responsible for providing high-quality patient care and coordinating services to ensure the best possible outcomes for our patients.
Key Responsibilities:- Provide direct patient care services to hospice patients and supportive care to the patient's family
- Act as primary liaison between physician, patient/family and hospice team
- Manage and collaborate with an interdisciplinary group to provide comfort, peace, and dignity
- Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
- Maintain all assigned patient clinical records
- RN license in the state of requested employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
- Current driver's license and automobile insurance
- Current CPR certification
