About the Role

We are seeking a highly skilled RN Clinical Liaison to join our team at New Century Hospice. As a key member of our organization, you will play a vital role in educating healthcare professionals about our hospice services and providing end-of-life solutions that prioritize clinical excellence, compassion, and dignity.

Responsibilities
  • Ensure effective and timely intake and referral processes to meet the needs of patients, families, and referral sources.
  • Complete admissions processes for referred patients in emergency cases, as staffing indicates, and for inpatient hospice admissions.
  • Provide accurate and timely information regarding hospice services to inquiring individuals.
  • Respond to calls from referral sources, initiate paperwork, and begin the intake/referral process.
  • Serve as a patient advocate in evaluating and developing plans of care and obtaining informed consent for hospice.
  • Engage with multiple account types, including SNFs, ALFs, hospitals, and physicians, to offer in-services and education on the benefits of hospice.
Requirements
  • A compassionate and empathetic approach to working with clients, patients, and families.
  • A valid RN license in the state(s) of employment.
  • A minimum of 2 years of healthcare experience, preferably in hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planning, hospice case management, or RN education.
  • Strong community connections and a respected reputation.
