About the Role:

  • Provide personalized care and support to seniors in their homes, assisting with daily living activities and promoting independence.
  • Develop and implement individualized care plans that meet the unique needs and goals of each senior.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support for seniors.
  • Monitor and report any changes in a senior's condition or behavior to the care team.
  • Document all interactions and care provided to seniors in accordance with company policies and procedures.

Requirements:

  • Degree in Nursing or related field (or equivalent experience).
  • Minimum 2 years' experience working with seniors in a home health care setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with seniors, families, and healthcare professionals.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation.

Salary: $50,000 - $70,000 per year, depending on experience.
